If you have reached retirement age,
Age Pension ensures you have an adequate income in your retirement. You
qualify for Age Pension if you are over a certain age; and meet the
residence requirements.

The Commonwealth
Senior Health Card helps
senior Australians with the cost of medicines.
You will get a discount on prescription medicines
though the Pharmaceutical Benefits Scheme (PBS).

Kohinoor is a
day activity program
which focuses on the individual needs of people and helps maintain and develop
their skills by providing group and communal activities outside their home.
Kohinoor also provides the chance for carers to take a break.

Persons Program offers companionship and support to people with an intellectual
disability, as well as a program of recreation and leisure that is sensitive to

Aged Care Assessment
The
Grampians Aged Care Assessment
Service is committed to the provision of skilled multi-disciplinary assessment
services. This will include provision of relevant information, advice and
referral, to ensure ease of access to necessary services and supports in order
to promote, improve or maintain the physical, mental emotional and/or social
wellbeing of older people or people with disabilities in the Grampians region.
Ballarat City Council Home Care Service
City of Ballarat Home Care Services provide in-home support to people of all
ages with disabilities and to frail aged people who wish to remain in their
own homes but require assistance to maintain their independence. Short-term
assistance for families in crisis may also be provided.

Community Aged Care
Package
Southern Cross Care (Vic)
recognises that, for many seniors, living in the comfort
of their own home is an important part of growing older.
Community Aged Care Packages (CACPs) are packages of planned and managed services to help older
people with complex care needs to remain living in their own homes. They are
designed for each individual and are based on their particular needs. To be
eligible to receive a Package, your needs must first be assessed by an Aged Care
Assessment Service (ACAS) to determine whether you need the kind of help a
package provides.
