Community Aged Care Package

Southern Cross Care (Vic) recognises that, for many seniors, living in the comfort of their own home is an important part of growing older. Community Aged Care Packages (CACPs) are packages of planned and managed services to help older people with complex care needs to remain living in their own homes. They are designed for each individual and are based on their particular needs. To be eligible to receive a Package, your needs must first be assessed by an Aged Care Assessment Service (ACAS) to determine whether you need the kind of help a package provides.
