Adult Mental Health Services (Grampians Psychiatric Services)
These services assist in providing Assessment, Treatment and Psychosocial Rehabilitation to people with a serious mental illness and associated psychosocial disability. The age group is typically between 16 and 65 years of age. Crisis Assessment and Treatment Triage Provides the first point of clinical contact with the service. The Triage worker has the role of listening to people who contact the service and assist them in meeting their needs. The Crisis Assessment and Treatment Team (CATT) Offer assessment and community based treatment (as an alternative to hospitalisation) 24 hours a day, 7 days a week. Continuing Care Team Provides service to the majority of people who access Public Mental Health Services. The CCT provide assessment, treatment and support via the case management system. Mobile Support and Treatment Services (MSTT) Provides intensive, ongoing support to clients who have a significant level of disability associated with mental illness. Acute Hospital Admissions The Adult Acute Admission Unit Provides care, treatment and assessment to adults and adolescents who require intensive support that cannot be provided in the Community. These have a key focus on working towards and maintaining recovery to improve the quality of life for all consumers. Programs offered include: Arts and Crafts, Computer and Internet Access, Gardening, Cooking, Leisure and Sporting, Relaxation, Woodwork and Restoration, Vocational Training, Young Persons Group, Drop in and Further Education Development. Home Based Outreach Support - Carers Provides high quality, individualised outreach support and psychosocial rehabilitation for participants in their own setting, to enhance their social and living skills and independence in the community. Aged Persons Mental Health Services (Grampians Psychiatric Services) These services assist people who have an existing mental illness or experience a mental illness in later life. The age group is typically 65 years of age and older.

Socialisation and Community Access: Men's Club Thursday Nights (joint program of Centacare & Grampians Psychiatric Services), Open house, Wednesdays 1pm - 4pm, Assistance with shopping, budgeting & financial management, Computer Skills. Planned Respite Program Aged primarily 16 to 64 years. Provides service to residents of the Central Highlands Sub-Region. Referrals are assessed for individual needs, provides timeout for families, respite accommodation available as required.

Self Harm Sovereign House Sturt Street, Ballarat Sovereign House is a 12 bed secure/extended care unit that provides a comprehensive rehabilitation program for those who have a serious recurrent psychiatric disorder. Steele Haughton Unit (Queen Elizabeth Centre) Dana Street Ballarat Geriatric Psychiatric Assessment and Treatment (Community Aged Persons Services) Intensive Outreach Service Provides care and support to older people over the age of 65 with a mental illness and complex needs who are at risk of homelessness. Psychogeriatric Care Unit Initiative Target group is people with dementia currently living in Nursing Homes, Hostel and Supported Residential Services whose care needs are not easily met due to challenging and difficult behaviours and severe cognitive defects. Geriatric Psychiatric Assessment and Treatment Team (GPATT) Assists residents of the Grampians Region who are 65 Years and older and experiencing a serious mental illness. Acute Hospital Admissions (Aged Persons Inpatient Services) Residential Services to those people over 65 years of age are provided at the Steele Haughton Unit. Three levels of care are available depending on need. Extended Care Unit Offers treatment and ongoing support to patients requiring specialist attention unavailable in nursing homes and other generic aged care facilities. The Acute Inpatient Unit Provides short term intensive support and treatment when such options are unattainable in the community. The Day Hospital Allows for intensive support and monitoring before and after admission.
